The disabled facility, at Richardson Dees Park in Wallsend, has been filled with grass and soil, and the parks team said on social media that the flush had also been broken and the door damaged.
The toilet, a Changing Places facility, was designed for severely disabled people and has extra equipment like hoists, adult changing space, and space for carers.
North Tyneside Council said repairs would be carried out "as quickly as possible" and the nearest facility was at Wallsend Library.
It is believed the toilet was targeted on Saturday evening, the council said, and had been reported to Northumbria Police, according to the Local Democracy Reporting Service.
Changing Places toilets are built near parks, train stations and shopping centres to provide extra facilities.
A council spokesperson said: "We would also like to thank members of the Wallsend community who have offered to help with the clean-up and repairs.
"The response and support for the park has been overwhelming and greatly appreciated."
