Millwall goalkeeper Lukas Jensen saved all four Queens Park Rangers penalties in a shootout to help the Lions progress in the Carabao Cup.
The first-round tie at Loftus Road finished 1-1 after normal time and went straight to spot-kicks - with Jensen proving the visitors' hero.
He denied four successive QPR takers while Jake Cooper and Mark Sykes slotted home their penalties to seal Millwall's place in the second round.
Millwall took the lead early in the second half with just their second attempt on goal when debutant goalkeeper Pierce Charles dropped Camiel Neghli's cross straight to Luke Cundle, who tapped in.
QPR took the tie into a penalty shootout four minutes from time when Jimmy Dunne's downward header from Ilias Chair's corner kick was deflected in off Lions defender Sykes.
After a minute's applause for ex-Millwall boss Kenny Jackett who died in June, QPR had started with intent in a lively opening, with Chair's early effort deflected just past the post by Casper de Norre's last-gasp block.
But it proved a false dawn in terms of goalmouth action, with both sides lacking sharpness and neither mustering an attempt on target before the break.
Boy Kemper almost put the hosts in front on his Hoops debut soon after the restart but his near-post header drew a point-blank stop from Jensen.
The contest's first attempt on target was followed less than a minute later by the second as Cundle accepted a gift from Charles, the Manchester City loanee goalkeeper, to give Millwall the lead.
QPR plugged away, with Jensen denying Chair one-on-one, and they equalised when debutant Sykes turned in Dunne's header seconds after his fine challenge had denied Alfie Lloyd as he bore down on goal.
The tie went to penalties and Charles kept out efforts from Neghli and Tairyk Arconte, but it was not enough as Jensen denied Chair, Harvey Vale, Koki Saito and Kwame Poku to send the Lions through to the next round.
After reaching last season's Championship play-off semi-finals, Millwall open their league campaign at Bristol City next Saturday, while QPR head to Portsmouth.
