The Isle of Man parliament voted 15 to 4 in favour of a bill to allow terminally ill adults the right to choose medical help to die.
The bill was introduced by Member of the House of Keys, John Rimmington and Quintin Gill after they learned of the plight of a local man, Patrick Kneen, 74, who suffers from prostate cancer.
Mr Kneed and his wife launched a "die with dignity" website where he says that if he deteriorates to the point where he is in severe pain he would rather die by assisted suicide than waste away.
Mr Rimmington said: "There are many circumstances in which it is the appropriate thing to do."
